GP2GP patient notes transfer - Proof of Concept (delivered by HISAC Office)

The GP2GP patient notes transfer project will deliver Practice Management System (PMS) capability for General Practitioners enabling a patient's clinical notes to be electronically transferred from their old GP when the patient is transferring their ongoing care.

Currently the project is scoped to include the following clinical information pertaining to the patient (in a structured message file):
  • Contact and Personal data (name, address etc)
  • Care Plus Statistics
  • Summary of ACC Claims
  • Immunisations and Injection events
  • Screening events
  • Diagnoses (medical history)
  • Consultation Notes
  • Prescribed Medications
  • Allergies and Warnings
  • Inbox documents such as lab results and letters

It is also intended that an unstructured PDF file will be transferred which will include all relevant clinical data, some of which may not have been transferred in the structured file. This will be able to be perused and/or cut and pasted by the new GP.

The project is not intended to deliver electronic patient notes transfer functionality for health providers who are not GPs. The project comprises three phases; a concept phase, a design phases and an implementation phase. The concept phase will culminate in a requirements document that has been agreed by the mainstream PMS vendors and the project's reference committee.

The reference committee comprises GPs and health sector experts to assist with advice concerning GPs business requirements, as well as medico legal and PMS technology issues. The concept phase is scheduled for completion in March 2008.

The design phase will deliver an interchange format and implementations guide which will enable PMS vendors to build exporting, importing and messaging routines into their software allowing the secure electronic transfer of clinical patient notes between different brands of PMS. It is currently scheduled for completion in June 2008.

The implementation phase is scheduled to take between four and eight months and will culminate in GP2GP notes transfer functionality being introduced in all mainstream GP PMS systems in New Zealand.
